Sie haben Ihren ersten Termin bei Chiro & Co, dem Zentrum für Chiropraktik und Osteopathie in Esslingen? Bitte laden Sie das passende Anmeldepaket herunter, füllen Sie es aus und bringen Sie es ausgedruckt zum ersten Termin mit – oder laden Sie die ausgefüllte Datei über den Link zum Terminmanagement in Ihrer Terminbestätigungs-E-Mail hoch.
Jedes Anmeldepaket enthält Anmeldung und Behandlungsvertrag, Aufklärungsbogen, Honorarverzeichnis, Anamnesebogen und die Datenschutzinformation. Wählen Sie das Paket nach dem Alter der Patientin oder des Patienten:

Für Patienten, die bereits bei uns in Behandlung sind – zur Aktualisierung Ihrer Angaben. Die Formulare enthalten den aktuellen Behandlungsvertrag, die Aufklärung, das Honorarverzeichnis und die Datenschutzinformation. Wie fülle ich die Formulare aus?
Bitte laden Sie das PDF herunter und öffnen Sie es in Adobe Acrobat Reader. Die Vorschau im Browser speichert Eingaben häufig nicht. Sie können das Formular am Computer ausfüllen oder ausdrucken und von Hand ausfüllen.
Wie gebe ich die Formulare ab?
Bringen Sie das ausgefüllte Formular ausgedruckt zu Ihrem Termin mit oder laden Sie die Datei über den Link zum Terminmanagement in Ihrer Terminbestätigungs-E-Mail hoch.
Wer unterschreibt bei Kindern und Jugendlichen?
Bei Patienten unter 18 Jahren füllt eine sorgeberechtigte Person das Anmeldepaket aus und unterschreibt es.
Gibt es die Formulare auch auf Englisch?
Ja. Die Anmeldepakete für Neupatienten stehen weiter unten auf dieser Seite auch in englischer Sprache bereit.

Is this your first appointment at Chiro & Co, the centre for chiropractic and osteopathy in Esslingen, Germany? Our registration packages for new patients are also available in English. Please download the package that applies to you, complete it and bring a printed copy to your first appointment – or upload the completed file via the appointment management link in your confirmation e-mail.
Each package contains the registration and treatment contract, the information and consent form, the fee schedule, the medical history form and our data protection information. Please choose the package according to the patient’s age:
New patient form – adults (PDF, fillable)
New patient form – babies and toddlers (0–4 years) (PDF, fillable)
New patient form – children (4–12 years) (PDF, fillable)
New patient form – adolescents (12–17 years) (PDF, fillable)
Please note: The English forms are convenience translations. In case of doubt or discrepancy, the German version shall prevail. Forms for existing patients are currently available in German only; our team will be happy to help you at the practice.
How do I fill in the forms?
Please download the PDF and open it in Adobe Acrobat Reader. Entries made in a browser preview are often not saved. You can complete the form on your computer, or print it and fill it in by hand.
Who signs for children and adolescents?
For patients under 18, a person with parental custody completes and signs the registration package.
